Job summary

Valeris is seeking a Care Coordinator to own patient cases from initiation to closure, handling benefits investigations, prior authorizations, and patient assistance navigation using Lynk technology. You will manage relationships with patients, prescribers, and client stakeholders to ensure compliant, efficient care.

The role requires attention to detail, strong communication, and the ability to work an 8.5-hour shift between 8 am and 9 pm ET, with remote work eligibility based on business

Responsibilities

• Ownership of patient cases from initiation to closure, including benefit investigations and prior authorizations.
• Assist patients through resolution via email and inbound/outbound calls using Lynk.
• Coordinate nurse teach with field-based nurse educators as applicable.
• Document all activities in the Lynk system per SOPs and business rules.
