Job summary

Valeris, Inc. is seeking a Care Coordinator to own patient cases from initiation to closure, including benefit investigations, prior authorizations, and patient assistance navigation. You will work with patients, prescribers, and care partners via email, phone, and our Lynk technology.

The role requires a high level of ownership, attention to detail, and a compassionate approach to helping patients access and adhere to medications; bilingual Spanish/English is a plus.

Qualifications

• Detail oriented with professional telephone etiquette.
• Able to multitask effectively and communicate clearly.
• Bi-lingual Spanish and English is a plus.

Responsibilities

• Owns patient cases from initiation to resolution, including benefit investigations and prior authorizations.
• Handles inbound/outbound calls and emails, guiding patients and healthcare providers.
• Coordinates nurse teaching and adherence services with field-based staff.
• Reports adverse events and documents activities in Lynk system.
