Job summary

Arkansas Provider Coalition is seeking an Enhanced Care Coordinator to support members with complex care needs from a remote/field-based location within Arkansas. You will develop and monitor PCSPs, coordinate across health and community resources, and work with care teams to ensure timely, coordinated services.

The role requires strong communication, organization, and problem-solving skills, plus travel across Arkansas and documentation in EMR systems.

Qualifications

• Two years of college-level coursework in social work, psychology, sociology, or a health-related field.
• At least two years of experience in care coordination.
• Bachelor’s degree preferred in social work, psychology, sociology, or health-related field.
• Experience coordinating multiple services for complex populations.
• Knowledge of care coordination practices and navigating complex service systems.
• Must possess valid Arkansas driver’s license and have reliable vehicle for travel.

Responsibilities

• Develop, implement, document, evaluate, and monitor PCSPs for members with complex care needs.
• Coordinate services across physical health, behavioral health, IDD, and community resources.
• Provide information, recommendations, and support for PCSP implementation.
• Monitor member circumstances and communicate changes to support timely adjustments.
• Conduct weekly care team calls to discuss needs and barriers.
• Facilitate communication among members and support networks to ensure continuity of care.
• Participate in care planning meetings and care transitions.
• Educate, advocate, and assist members and families to navigate services.
