Responsibilities

• Outreach to members by phone or home visits to discuss care plans.
• Coordinate care activities with providers and family members.
• Answer member and provider inquiries about care plans.
• Communicate with care managers and practitioners to ensure continuity.
• Document and maintain member records per state guidelines.
• Identify needs and refer to Care Manager or community resources.
