Job summary

Arkansas Provider Coalition (APC) is seeking a Care Coordinator I to support Arkansans by coordinating services across health and community resources, with emphasis on IDD, behavioral health, Medicaid, and waiver programs.

The role involves developing person-centered service plans, engaging families, traveling to member homes, and documenting in the EMR while ensuring regulatory compliance in a field-based setting.

Qualifications

• Bachelor’s degree in social work, psychology, sociology, or health-related field.
• Experience in care coordination, case management, Medicaid/waiver programs, or DHS/DCFS experience.
• Ability to navigate complex health and social service systems.
• Strong advocacy, communication, and organizational skills.

Responsibilities

• Develop and maintain person-centered service plans based on member needs.
• Coordinate services across behavioral health, IDD/waiver, and community resources.
• Advocate for members and maintain regular contact with families and providers.
• Maintain documentation in EMR within required timeframes.
• Travel regularly for field-based visits and meetings.
