Job summary

CareSource is seeking a Community Health Worker to join its inter-disciplinary care team in Arkansas. You will coordinate care for members, engage with them in various settings, accompany them to appointments, and help execute individualized care plans.

You will travel regularly to member, provider, and community sites, verify enrollment, gather information, and support health education. Strong communication and compassion are essential.

Qualifications

• Requires strong communication and interpersonal skills.
• Proficient with Microsoft Office (Outlook, Word, Excel).

Responsibilities

• Engage with members to establish professional relationships in diverse settings.
• Accompany members to appointments and social service encounters as needed.
• Coordinate logistics to support care plan goals including reminders, transportation, and childcare.
• Verify eligibility, enrollment history, demographics and health status for each member.
• Contribute to assessments by gathering information from members, families, providers, and other stakeholders.
• Assist with developing and implementing individualized care plans and reporting to the Case Manager.
• Identify and manage barriers to achieving care plan goals and empower members.
• Assist with health education and wellness materials as directed by Case Manager or Team Lead.
• Maintain documentation per Care Management protocols and guidelines.
• Travel regularly to member, provider, and community sites to ensure program effectiveness.
