Qualifications

• Minimum qualifications include a degree in a social/health field or equivalent experience.
• Experience with care coordination, behavioral health, IDD, Medicaid/waiver programs, DHS/DCFS is valued.
• Strong written and verbal communication and advocacy skills.

Responsibilities

• Develop, document, evaluate, monitor, and modify annual PCSPs based on member needs.
• Coordinate services across behavioral health, IDD/waiver, and community resources.
• Advocate and maintain contact with members, families, and providers.
• Identify barriers to care and connect to appropriate resources.
• Monitor service delivery and update PCSP as needs change.
• Maintain regular contact and conduct required in-person visits.
• Document activities in EMR within required timelines and ensure regulatory compliance.
• Collaborate with providers, state agencies, and community partners to coordinate care.
• Keep current on Medicaid/welfare programs and ensure quality improvements.
