Job summary

Pro Care Innovations is seeking a motivated Care Team Coordinator to join our Chronic Care Management team. This full-time, remote role requires residency in Benton or Washington County, Arkansas, and focuses on patient-centered CCM services.

You will build relationships with CCM patients, conduct monthly outreach, document interactions, coordinate with providers, and ensure HIPAA compliance. The position offers production-based compensation with a guaranteed minimum and growth opportunities

Qualifications

• Must reside in Benton or Washington County, AR to meet program eligibility.
• Medical Assistant certification or equivalent healthcare experience required.
• Strong communication and relationship-building skills.
• Excellent organizational skills and attention to detail.
• Ability to work independently while managing a patient caseload.
• Comfortable with technology and electronic documentation systems.
• Experience in CCM, care coordination, case management, primary care, or population health is desirable.
• Passion for helping patients improve health outcomes through ongoing engagement.

Responsibilities

• Build and maintain relationships with an assigned panel of CCM patients.
• Conduct monthly patient outreach and care coordination activities.
• Document patient interactions accurately and timely in CCM records.
• Monitor patient needs, care plan updates, and follow-up items.
• Address patient questions with professionalism and compassion.
• Coordinate with providers and internal teams to support continuity of care.
• Ensure documentation meets regulatory and billing compliance standards.
• Support patient retention and engagement within the CCM program.
• Maintain confidentiality and HIPAA compliance.
