Arkansas Provider Coalition, LLC is hiring a remote Care Coordinator to support members across Arkansas counties. You will develop and monitor personalized plans, connect members with services, and collaborate with families and providers to ensure timely, appropriate care.

Ideal candidates bring 2+ years in care coordination, strong communication, and proficiency with MS Office. This role requires travel for meetings and trainings and offers full-time employment with non-exempt status.

Qualifications

• Minimum 2 years of college coursework in social work or related field; bachelor’s preferred.
• 2+ years of care coordination experience; familiarity with managed care a plus.
• Proficient with MS Office (Excel, Outlook, Word) and confidential data handling.

Responsibilities

• Develop, implement, and monitor person-centered plans for diverse member needs.
• Coordinate with members, families, and providers to remove access barriers to care.
• Maintain regular contact with members and care teams; document in EMR within 24 hours.
