Job summary

Western Healthcare Alliance is seeking a Care Navigator to support care coordination across Medicare/Medicaid populations. You will work with an interdisciplinary team, organizing health and social information via phone outreach, while ensuring culturally sensitive communication and adherence to program guidelines.

The role emphasizes care transitions, quality outcomes, and collaboration with clinics for education, recruitment, and resource linkage.

Qualifications

• Two years of progressive related experience working with diverse populations; healthcare setting preferred.
• Bachelor’s degree in related field and/or related certification (MA, CNA, LPN) preferred, or equivalent combination.
• Knowledge of health education, motivational strategies, and working with underserved populations.

Responsibilities

• Work collaboratively to support individualized care plans addressing physical, behavioral, and social needs.
• Exhibit competence in the Seven Domains of Care Coordination in the primary care setting: Population Health Management; Comprehensive Assessment and Care Planning; Interpersonal Communication; Education/Coaching; Health Insurance knowledge; Community Resource knowledge; Research and Evaluation.
