WellSense Health Plan is seeking a Care Coordinator to join the Care Management team. This remote, full-time role engages members by outreach and assessments, coordinates access to medical and community services, and supports positive health outcomes.

Qualifikationen

• Associate degree in Healthcare, Human Services, Social Work, Psychology, Public Health, or related field required; or equivalent.
• Bachelor's degree in a related field preferred.
• Minimum 2 years in healthcare administration, care coordination, or related roles.
• Experience with Medicaid populations preferred.
• Bilingual skills preferred.

Aufgaben

• Engage members via telephonic or in-person outreach to identify needs and enroll in care management.
• Educate members about benefits, services, and community resources.
• Coordinate access to medical, behavioral health, and community services.
• Facilitate referrals and connections to providers and state agencies.
• Advocate to reduce barriers to care and address social determinants of health.
• Support medication access, appointments, and treatment adherence.
• Triage calls and referrals; support care management workflows.
• Maintain accurate documentation and prepare reports.
• Participate in team meetings and quality improvement activities.
