Job summary

WellSense Health Plan is seeking a Care Coordinator to engage members by phone or in-person visits, assess non-clinical needs, and coordinate access to healthcare, behavioral health, and community services.

You will work with clinical staff, providers, and community partners to remove barriers and promote positive health outcomes through motivational interviewing and member-centered engagement across multiple programs.

Qualifications

• Associate degree in Healthcare, Human Services, Social Work, Psychology, Public Health, or related field required; or equivalent.
• Bachelor's degree in related field preferred.
• Two (2) years of healthcare administration, care coordination, customer service, or related healthcare support experience.
• Bilingual skills preferred.

Responsibilities

• Engage members via telephonic or face-to-face outreach; assess non-clinical and social needs.
• Coordinate access to medical, behavioral health, and community-based services.
• Facilitate referrals and connections to providers, vendors, and state agencies.
• Advocate to reduce barriers to care and address social determinants of health.
• Support medication access, appointments, and adherence.
• Collaborate with interdisciplinary teams for coordinated care.
• Maintain up-to-date knowledge of community resources.
