Greater Good Health is seeking a Care Navigator to support patients in the Mountain Time zone from a remote home office. You will handle outbound outreach, schedule AWVs, coordinate with Nurse Practitioners, and ensure timely follow‑ups and care transitions.

Qualifications

• 1–3 years of experience in a patient-facing healthcare role.
• Experience with standard operating systems (Windows, Word, Excel, Internet).
• Ability to communicate complex health information empathetically.
• Solid PHI handling and HIPAA compliance awareness.

Responsibilities

• Conduct proactive outreach to schedule AWVs and telehealth visits.
• Coordinate with NPs for day-to-day care coordination and scheduling.
• Retrieve and reconcile medical records from outside providers.
• Follow up on outstanding orders and ensure timely results routing.
• Maintain thorough documentation of patient interactions.
