Job summary

WellSense Health Plan is seeking a Care Coordinator to engage members via telephonic or face-to-face outreach, assess non-clinical needs, and coordinate access to healthcare and community services. You will collaborate with clinicians, providers, and community partners to support care management programs and improve outcomes.

Qualifications

• Associate degree in Healthcare, Human Services, Social Work, Psychology, Public Health, or a related field required; or equivalent combination of education and relevant experience.
• Bachelor's degree in a related field preferred.
• Two (2) years of experience in healthcare administration, care coordination, customer service, call center operations, community services, or related healthcare support functions.
• Experience working with Medicaid populations preferred.
• Bilingual skills preferred.

Responsibilities

• Engage members via telephonic or face-to-face outreach and assess non-clinical and social needs.
• Coordinate access to medical, behavioral health, and community-based services.
• Utilize motivational interviewing to enhance member engagement and participation in healthcare activities.
• Educate members regarding benefits, services, and community resources; provide appointment reminders and care outreach.
• Triage cases, referrals, and member needs to clinical staff and internal departments; support care management workflows.
