Qualifications

• 3+ years of relevant community outreach, facilitation, or healthcare experience.
• CMS-mandated training and state-applicable requirements.
• High level of cultural competency for disenfranchised populations in New Jersey and New Mexico.

Responsibilities

• Execute person-centered care plans for patients with high-risk illnesses (PIN).
• Bridge between patient and billing practitioner; support care coordination.
• Assess social determinants of health and set actionable goals.
• Navigate healthcare system; assist with transportation and telehealth access.
• Coordinate with tech team to update patient charts in CMS with accuracy.
• Refer patients to community resources and maintain partnerships with local organizations.
• Provide emotional support and education to patients and families.
• Document interactions and ensure compliance with Medicare services.
