CenterWell Senior Primary Care seeks a Care Coach to provide proactive, patient-centered care coordination and social needs support for high-risk patients. You will serve as the primary contact, conduct home visits, and foster connections with primary care, specialists, pharmacies and home health to close care gaps.

You will travel to patients' homes and clinics, conducting outreach, coordinating care plans, and delivering culturally competent education.

Formación

• Healthcare professional with 3+ years direct patient care.
• Ability to discuss chronic conditions and reinforce medication instructions.
• Comfortable with regular home visits and community outreach.
• Experience in patient education, care coordination, and social support for high-risk or geriatric populations.

Responsabilidades

• Coordinate care across health and social service systems as patient advocate and clinical support.
• Perform home visits to assess living conditions and barriers to engagement.
• Identify social needs and connect patients with community resources.
• Deliver chronic disease education and reinforce provider recommendations.
• Coordinate care transitions and communicate across care settings.
