Job summary

Humana Inc is seeking a Telephonic Nurse Care Manager to support patients remotely, coordinating post-acute services and admissions. You will assess needs, educate on care options, and work with the clinical team to ensure timely and appropriate care transitions.

Requires an Associate's Degree with BSN preferred, a compact state RN license, and 3+ years in post-acute care. Weekend training and occasional travel may be required, with a 20-hour weekly schedule on weekends.

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license; additional state licenses may be needed.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Strong analytical and communication skills with empathy over the phone.

Responsibilities

• Be a CenterWell Home Health representative to support patients post-discharge or needing post-acute services.
• Provide options and education on care settings, Medicare coverage, billing, and resources.
• Make clinical level of care determinations based on discussions and records.
• Assist admissions and navigate referrals to maximize timely admission conversions.
• Follow up on referrals and admissions; ensure patient satisfaction and alignment with goals.
• Adhere to HIPAA, ethics, and company policies; educate about Home Health, Hospice, and Palliative Care.
