You will collaborate with the clinical team, ensure timely follow-up, and uphold HIPAA and company policies while delivering empathetic, phone-based care.

Qualifikationen

• Associates Degree required; BSN preferred.
• Compact state RN license is required.
• At least 3 years post-acute nursing experience.
• Home health or hospice experience preferred.
• Knowledge of home health, hospice and palliative care services.
• Strong verbal and interpersonal skills with empathetic phone communication.

Aufgaben

• Serve as a telephonic RN Care Manager for post-discharge patients.
• Provide education on post-acute options, services and plans.
• Make clinical level of care determinations and coordinate referrals.
• Follow up and track referral and admission outcomes.
• Ensure patient satisfaction and adherence to policies and HIPAA.
• Assist admissions as needed and support right-care placement.
• Collaborate with clinical teams to optimize care transitions.
