Job summary

CenterWell Home Health is seeking a Telephonic Nurse (Care Manager) to support patients transitioning into and out of our services. You will liaise with the clinical team to facilitate follow-up and ensure appropriate post-acute care options, including home health, hospice and palliative care.

The role requires an Associate's degree, a compact state RN license, and at least three years of post-acute nursing experience, with home health or hospice knowledge.

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license required.
• At least 3 years post-acute nursing experience; home health or hospice experience preferred.
• Knowledge of home health, hospice, and palliative care services.
• Excellent verbal and interpersonal skills; empathy in phone communication.

Responsibilities

• Be a CenterWell Home Health representative, assisting patients discharged from service or needing post-acute services.
• Provide patient options and education using multiple tools, addressing location lookup and inquiries.
• Make clinical level of care determinations based on discussion, records and data.
• Follow up and track referral and admission outcomes to meet satisfaction goals.
• Assist admissions by acting as an ambassador for patients meeting admission requirements.
• Focus on placing the right patient to the right care setting at the right time.
• Adhere to mandatory training including HIPAA, ethics and company policies.
• Provide education on Home Health, Hospice, and Palliative Care services.
• Participate in special projects and other duties as assigned.
