Responsibilities

• Serve as primary Care Manager for an assigned patient panel and enroll eligible patients.
• Perform comprehensive assessments and establish initial Care Management needs.
• Develop and update individualized care plans and conduct monthly reviews.
• Coordinate care with providers, caregivers, and interdisciplinary team members.
• Document all Care Management services accurately and timely.
• Escalate clinical concerns to the appropriate care team member.
