Job summary

Esrun Health is seeking a Remote Care Manager to oversee patient panels, perform care coordination, and drive billable care management encounters. The role emphasizes adherence to care plans, motivational coaching for chronic disease management, and reliable documentation in HIPAA-compliant workflows.

The position offers autonomous, flexible work environments, with performance-based compensation tied to patient encounters and monthly milestones.

Qualifications

• Graduates from accredited Schools of Nursing (LPN, LVN, RN, BSN, etc.)
• Current COMPACT license to practice as RN/LVN/LPN with residence state
• Minimum 2 years of clinical experience in clinic, Med/Surg, Case Management or home health care
• Hands-on experience with Electronic Medical Records and HIPAA-compliant workflows

Responsibilities

• Abide by the plan of care and orders of the practice
• Provide prevention and intervention for chronic disease through motivational coaching
• Develop positive interactions with patients on behalf of the practices
• Improve revenue by creating billable Care Management episodes
• Develop detailed care plans for doctors and patients
• Understand health care goals for chronic disease management
• Attend monthly clinical updates and supervisor meetings
