Job summary

Pro Care Innovations is seeking a motivated Care Team Coordinator to support our Chronic Care Management program. This role focuses on building patient relationships, coordinating care, and ensuring accurate documentation while working remotely.

The position requires Medical Assistant certification or equivalent healthcare experience, strong communication, and the ability to manage a patient caseload with minimal supervision.

Qualifications

• Medical Assistant certification or equivalent healthcare experience required.
• Strong communication and relationship-building skills.
• Excellent organizational skills and attention to detail.
• Ability to work independently while managing a patient caseload.
• Comfortable with technology and electronic documentation systems.

Responsibilities

• Build and maintain ongoing relationships with CCM patients across assigned panel.
• Conduct monthly patient outreach and care coordination activities.
• Document patient interactions timely and accurately per CCM requirements.
• Monitor care plans, updates, and follow-up items.
• Coordinate with providers and internal teams to support continuity of care.
• Ensure documentation meets regulatory and billing standards.
• Support patient retention and engagement within CCM program.
• Maintain HIPAA confidentiality.
