Job summary

UPHP seeks a Remote Community Based Clinical Coordinator - Care Manager to perform care management functions, coordinating medical and supportive services for eligible members. This role requires in-person member visits and collaboration with care teams to ensure high-quality, cost-effective care.

Responsibilities include assessment, care planning, and coordination with providers, home health, and community resources, while maintaining confidentiality and adherence to MDHHS, CMS, DIFS, and NCQA

Qualifications

• Licensed registered nurse in Michigan.
• Bachelor of Science in Nursing or social work degree preferred.
• Two years clinical or health-related experience as RN or social worker.
• Two years managed care experience or five years of clinical RN/SW experience preferred.
• Strong communication and organizational skills.
• Valid driver’s license with proof of insurance; working vehicle.

Responsibilities

• Performs care management to assess, plan, and coordinate services across the continuum of care.
• Conducts required in-person visits with members in homes and nursing facilities.
• Develops plans of care with specific objectives and interventions.
• Monitors delivery of services and referrals to support the care plan.
• Collaborates with teams to ensure appropriate discharge and care coordination.
