Job summary

Millennium Physician Group is seeking a Care Coordinator to support care management in a primarily remote setting. The role collaborates with RNs, PCPs, and patients to reduce readmissions and improve outcomes.

The ideal candidate has an LPN license or CMA/RMA certification, strong care coordination experience, and solid documentation skills. Telephonic outreach, chart reviews, and cross-functional coordination are core duties.

Qualifications

• Active Florida LPN license or CMA certification from an accredited program (preferred).
• Experience in care coordination or population health is a plus.
• Proficient in using electronic health records and coordinating with care teams.

Responsibilities

• Assist the RN Care Manager in implementing and monitoring individualized patient care plans.
• Perform monthly patient chart reviews to identify care gaps and intervention opportunities.
• Monitor follow-up appointments, referrals, and care plan goals.
• Coordinate services among PCPs, specialists, hospitals, and community resources.
• Maintain accurate documentation in the EHR and track care management activities.
