CVS Health is seeking a Care Management Associate to support comprehensive coordination of medical services, including Care Team intake, screening, and the implementation of care plans to promote efficient utilization of healthcare resources. You will help identify needs, coordinate referrals, and monitor cases to ensure quality outcomes.

• 2–4 years of experience in healthcare field, medical/health setting, medical billing, and coding
• Experience in call center or customer service environment
• Works independently and competently, meeting deliverables and deadlines while demonstrating an outgoing, caring presence telephonically
• Demonstrates ability to meet daily metrics with speed, accuracy, a positive attitude, and strong written and oral communication skills
• Ability to work efficiently across multiple systems simultaneously, including Microsoft Office 365 applications, clinical platforms, and health plan systems
• Technically proficient, with ability to navigate computer systems and digital tools efficiently
• Ability to work independently while collaborating effectively with clinical teams
• Excellent communication skills for interacting with providers and internal stakeholders
• Commitment to delivering high-quality service and supporting patient care outcomes.

Responsabilidades

• Coordinate intake and triage for Care Team tasks and support implementation of care plans.
• Identify admission drivers and apply intervention assessment tools.
• Refer to medical services and coordinate required services under benefit plan.
• Monitor cases and refer outliers to clinical staff for discharge planning.
• Utilize Med Compass and CVS systems to build and research patient information.
• Support development and implementation of care plans under nurse or medical director supervision.
