Job summary

CVS Health is seeking a Care Management Associate to support comprehensive coordination of medical services, from intake through care plan implementation, and to promote efficient utilization of healthcare resources. The role involves triage, referrals, and collaboration with clinical staff to ensure high-quality patient care.

You will work with Med Compass and other CVS systems, maintain strong communication with providers, and contribute to care plan development in a remote, home-based setting

Qualifications

• 2-4 years of experience in healthcare field, medical/health setting, medical billing, and coding.
• Experience in call center or customer service environment.
• Ability to work independently while meeting daily metrics and maintaining quality communication.
• Proficiency with Microsoft Office 365 applications and clinical platforms.

Responsibilities

• Coordinate Care Team intake and triage; support care plan development and implementation.
• Identify and refer needed medical services; coordinate services per benefit plan.
• Promote effective utilization of healthcare services and improve care quality.
• Utilize CVS systems (e.g., Med Compass) to build and update member information.
• Support non-medical research for open cases and assist team members.
