Job summary

CVS Health is hiring for a Case Management professional to coordinate care, educate members, and support healthcare outcomes across the region. The role emphasizes collaboration with case managers, medical directors, and health programs to overcome barriers and improve care plans.

The ideal candidate will have a health care or human services background, CCM certification (preferred), strong communications and documentation skills, and the ability to travel within the specified region.

Qualifications

• Degree in Health Care/Human Services related field required.
• Case Management Experience preferred.
• Strong customer service orientation and problem solving skills.
• Excellent communication/telephonic skills.
• Excellent motivational interviewing skills; ability to build rapport telephonically.
• Attention to documentation and regulatory standards.
• Highly organized and self-driven with minimal supervision.
• Ability to travel within 125 miles for 1 day.
• Flexible scheduling including evenings.

Responsibilities

• Consult with case managers and health program staff using a holistic approach to overcome barriers.
• Present cases at conferences to obtain multidisciplinary review.
• Identify and escalate quality of care issues.
• Demonstrate negotiation to secure appropriate options and services.
• Deliver motivational interviewing to promote healthy behaviors and engagement.
• Provide coaching and information to empower member decisions.
• Encourage member participation with providers in decision making.
• Perform comprehensive evaluations of referred members and recommend case resolution approach.
