Job summary

CVS Health is seeking a Care Management Associate to support coordination of medical services, care team intake, and implementation of care plans under the direction of a nurse or medical director.

You will utilize Med Compass and other systems to build and enter member information, communicate with providers, and support development of comprehensive care plans. This full-time role offers benefits and remote work within the U.S.

Qualifications

• 2-4 years of experience in healthcare settings, medical billing or coding.
• Experience in call center or customer service environment.
• Ability to work independently and meet daily metrics with strong communication skills.
• Proficient with multiple systems and Microsoft Office 365.
• Technical proficiency to navigate clinical platforms and health plan systems.

Responsibilities

• Support coordination of medical services including Care Team intake and care plan implementation.
• Review and triage Care Team tasks and identify referrals to medical staff.
• Coordinate and communicate with providers and internal teams to ensure timely care delivery.
