Job summary

You will work with members, providers, caregivers, and care teams to support wellbeing and improve outcomes. This remote role, reporting to the Associate Director, Consumer Experience, requires experience in patient support, care coordination, and EMR data entry.

Qualifications

• High School Diploma or GED required.
• Experience providing in-center or virtual patient support.
• Experience in patient care coordination or similar healthcare setting.
• Experience with data entry and EMR systems.
• Ability to work in EST or CST time zones.

Responsibilities

• Coordinate administrative, operational, and customer support activities for care management programs and services.
• Collaborate with members, providers, caregivers, and care teams to support wellbeing and inform care decisions.
• Use discretion and judgment to prioritize requests and adapt procedures.
