Qualifications

• High School Diploma or GED.
• Experience providing in-center or virtual support or interaction with patients.
• Experience in patient support, care coordination, or a related healthcare setting.
• Experience with data entry and maintaining information within an EMR system.
• Can work EST or CST.

Responsabilités

• Coordinate administrative, operational, and customer support activities that back care management programs and healthcare services.
• Collaborate with members, providers, caregivers, and care teams to support member wellbeing through care coordination, patient record review and maintenance, and the preparation of care summaries that inform care teams, guide next steps, and improve patient outcomes.
• Use discretion and judgment in prioritizing requests, interpreting and adapting procedures, and applying established techniques.
• Use prior experience of care management processes, healthcare operations, and organizational practices to perform the role.
