Job summary

Humana Inc. is seeking a Care Management Assistant to support care management programs and healthcare services in a remote capacity.

You will coordinate members' care needs, perform advanced administrative tasks, and report to the Associate Director, Consumer Experience. You will collaborate with members, providers, caregivers and care teams to support wellbeing through care coordination, patient record review and care summaries that inform next steps and improve outcomes.

Qualifications

• High School Diploma or GED required.
• Experience in in-center or virtual patient support.
• Experience in patient support, care coordination, or healthcare setting.
• Experience with data entry in an EMR system.
• Ability to work in EST or CST time zones.

Responsibilities

• Coordinate assessment, coordination and facilitation of members' care needs.
• Collaborate with members, providers, caregivers and care teams to inform care plans.
• Exercise discretion in prioritizing requests and applying procedures.
• Apply experience in care management to perform the role.
