Cosán Group is seeking a compassionate Care Coordinator for a 100% work-from-home role in the United States. You will manage a large patient panel, provide monthly outreach, and deliver health coaching across multiple platforms.

A CMA/MA-C or CPhT certification is required, with at least 1 year of clinical experience and proficiency in EMR systems. You will collaborate with clinical teams, advocate for patients, and ensure HIPAA-compliant documentation.

Qualifications

• Must hold CMA, RMA, CCMA, NCMA, CMAC, or equivalent MA-C certification.
• OR Certified Pharmacy Technician (CPhT).
• Minimum 1 year of clinical experience in primary care, hospital, or a specialty healthcare setting.
• Experience with EMRs and strong computer navigation.
• HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload.
• Must be legally authorized to work in the United States.

Responsibilities

• Manage an assigned panel of 400-450 patients with monthly outreach to ensure continuity of care.
• Conduct 50–60 daily outbound calls for care plan support and health coaching.
• Maintain call quality and professional communication in every interaction.
• Collaborate with clinical teams, providers, and caregivers to address clinical and social needs.
• Close gaps in care through Chronic Care Management, Behavioral Health Integration, and RPM services.
• Advocate for patient needs by listening, investigating, and communicating solutions.
• Document all patient interactions in real time using the care coordination platform and maintain HIPAA compliance.
