Job summary

Cosán Group is seeking a compassionate Care Coordinator to manage a panel of 400–450 patients from a 100% work-from-home setup in the United States. You will conduct monthly outreach, provide health coaching, and collaborate with clinical teams to close gaps in care while maintaining HIPAA-compliant documentation.

The role requires a CMA/RMA/CCMA/NCMA/CMAC or CPhT certification, at least 1 year of clinical experience, and proficiency with EMR systems and multiple technology platforms.

Qualifications

• Must hold CMA/RMA/CCMA/NCMA/CMAC or equivalent MA-C certification.
• Certified Pharmacy Technician (CPhT) certification is also accepted.
• Minimum 1 year of clinical experience in primary care, hospital, or specialty healthcare setting.
• Experience with EMRs and ability to navigate multiple technology platforms.
• HIPAA-compliant home office with broadband requirements (50 Mbps down / 5 Mbps up).
• Legal authorization to work in the United States.

Responsibilities

• Manage an assigned panel of 400-450 patients and monthly outreach.
• Conduct 50–60 daily outbound calls for care plan support and health coaching.
• Coordinate with clinical teams to address medical and social needs.
• Advocate for patient needs and collaborate with providers and caregivers.
• Document all patient interactions in real time and maintain HIPAA compliance.
