Job summary

Cosán Group is hiring a Care Coordinator for a 100% work-from-home role in the United States. You must reside in the U.S. and have a private, HIPAA-compliant home office dedicated to work. The role centers on patient outreach, health coaching, and care coordination.

The ideal candidate has a CMA/RMA/CCMA/NCMA/CMAC or CPhT certification, at least 1 year of clinical experience, and EMR familiarity. You will support chronic care management and collaborate with clinical teams to close care gaps.

Qualifications

• Must hold CMA/RMA/CCMA/NCMA/CMAC or equivalent MA-C certification.
• Or Certified Pharmacy Technician (CPhT).
• Minimum 1 year of clinical experience in primary care, hospital, or a specialty healthcare setting.
• Experience with EMRs and strong computer navigation skills.
• HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability.
• Must be legally authorized to work in the United States.

Responsibilities

• Manage an assigned panel of 400-450 patients and conduct monthly outreach.
• Conduct 50-60 daily outbound calls to provide care plan support and health coaching.
• Maintain call quality standards with professional communication in every patient interaction.
• Collaborate with clinical teams, providers, and caregivers to address clinical and social needs.
• Document all patient interactions in real time using the care coordination platform.
