Valeris is seeking a Care Coordinator to manage patient cases from initiation to closure, including benefit investigations, prior authorizations, and navigating assistance programs. You will engage patients via phone and email, applying clinical knowledge and program rules to drive timely resolutions.

The role emphasizes ownership, attention to detail, and patient-centric service, with remote work eligibility and a typical 8.5-hour shift during business hours.

Qualifications

• Bachelor's degree strongly preferred or equivalent experience.
• Excellent communication and multitasking abilities.
• Basic computer literacy and comfort with technology.

Responsibilities

• Builds trusted relationships with patients, prescribers and client stakeholders, ensuring timely, accurate deliverables.
• Performs program welcome calls to patients and follow-up calls explaining coverage options.
• Manages inquiries and triages patients to internal or external resources as appropriate.
• Provides personalized case management and communicates benefit coverage steps to patients and HCPs.
• Documents all activities in the Lynk system and adheres to SOPs and business rules.
• Ensures adherence to training deadlines and company policies.
