Valeris is seeking a Care Coordinator to manage patient cases from initiation to closure, including benefit investigations, prior authorizations, and financial qualification determinations. You will support patients and healthcare providers via phone and email, ensuring timely resolution and adherence to program rules.

The role emphasizes ownership, attention to detail, and patient-centered care, with remote eligibility based on company criteria.

Qualifications

• Bachelor's degree strongly preferred or equivalent experience required.
• Customer service and healthcare industry experience is preferred but not required.
• Bi-lingual skills in Spanish and English are a plus.
• Ability to work an 8.5-hour shift between 8 am to 9 pm, Monday to Friday.

Responsabilités

• Own patient cases from initiation to closure, including benefit investigations and prior authorizations.
• Assist patients and callers via email and calls, providing resolution.
• Determine financial qualifications for assistance programs and assist patients accordingly.
• Coordinate nurse teaching with field-based nurse educators, as applicable to program.
• Document all activities in the Lynk system per SOPs.
