Job summary

Cosán Group is seeking a compassionate Care Coordinator to join our remote US-based team. You will manage a panel of 400-450 patients, perform monthly outreach, and deliver health coaching aligned to individual needs, from a home office that meets HIPAA standards.

Ideal candidates hold a CMA/RMA/CCMA/NCMA/CMAC or Certified Pharmacy Technician credential, with 1 year of clinical experience and EMR proficiency. A US work authorization is required.

Qualifications

• Must hold a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent state-issued MA-C).
• OR Certified Pharmacy Technician (CPhT).
• Minimum 1 year of clinical experience in primary care, hospital, or a specialty healthcare setting.
• Experience with EMRs and strong computer navigation skills.
• HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload.
• Must be legally authorized to work in the United States.

Responsibilities

• Manage an assigned panel of 400-450 patients and conduct required monthly outreach to ensure continuity of care.
• Conduct 50-60 daily outbound calls to patients, providing care plan support and health coaching.
• Collaborate with clinical teams, providers, and caregivers to identify and address clinical and social needs.
• Close gaps in patient care through Chronic Care Management, Behavioral Health Integration, and Remote Physiological Monitoring.
• Document all patient interactions in real time using the care coordination platform and maintain HIPAA compliance.
