Job summary

Tri-County Care is seeking a Care Manager to collaborate with individuals with intellectual/developmental disabilities, families, OPWDD, and providers to ensure safe, happy, and fulfilling lives. You will identify goals, build plans, and assist with applications to services to achieve person-centered outcomes.

The role supports members of all ages and requires a valid driver's license for travel to visits, with remote work and occasional regional office work possible.

Qualifications

• Bachelor's degree with two years of relevant experience, OR
• RN license with two years relevant experience, which can include any employment experience and is not limited to case management/service coordination duties, OR
• Master's degree with one year of relevant experience
• MSC Service Coordinators prior to July 1, 2018 are “grandfathered” to facilitate continuity of care

Responsibilities

• Work with individuals with intellectual/developmental disabilities and families to set goals.
• Identify goals and build plans to reach them.
• Assist with applying to supports and services to reach person-centered outcomes.
• Advocate with schools and medical providers to support clients.
