Tri-County Care is seeking a Care Manager to work with individuals with intellectual/developmental disabilities, families, OPWDD, and providers to ensure safe, happy, and fulfilling lives. You will identify goals, build plans, and assist with applying to supports and services for person-centered outcomes.

The role serves members of all ages, advocating for school involvement, locating medical providers, and aiding applications to Social Security, SNAP, Waiver programs, etc.

Formación

• Excellent written and verbal communication required.
• Strong conflict-management and problem-solving abilities.
• Proficiency with Microsoft Office suite (Word, Excel, Outlook).
• Ability to multi-task, organize, and manage time effectively.
