Qualifications

• Minimum of 1 year of experience in care coordination.
• Beginner-level experience with Microsoft Office tools.
• Preferably Licensed Practical Nurse (LPN).

Responsibilities

• Develop and manage annual Person Centered Support Plans (PCSPs).
• Ensure members’ health and service needs are met.
• Document member information in the EMR within 24 hours.

Education

Completion of 2 years of college-level coursework

Bachelor’s degree in social work, psychology, sociology, or health-related field
