Job summary

UnitedHealth Group is expanding its care management team in Caldwell, ID. The Care Coordinator will manage a panel of members with chronic health needs, performing comprehensive needs assessments and coordinating medical, behavioral, and social services to improve outcomes.

The role supports a person-centered approach, with flexibility for remote work and collaboration with internal teams and community partners. Idaho licensure and SDoH experience are required.

Qualifications

• Current and unrestricted Idaho license (LCSW, LMSW, LSW, LCPC, LPC, LMFT, LAMFT or LPN/LVN)
• 1+ years of Medicaid/Medicare or SDoH experience
• Access to a dedicated home workspace securing PHI

Responsibilities

• Serve as the primary care manager for dual eligible members
• Conduct comprehensive needs or wellness assessments for members
• Collaborate with care team and community resources to implement care plans
• Assist members with HCBS supports and referrals
• Support discharge planning and care transitions
• Educate and coach members for self-management
• Advocate for members to ensure needs are represented
• Assist nonclinical questions and route to Health Plan resources
• Engage members in person or by telecommunication to promote health outcomes
