Job summary

UnitedHealth Group in Meridian, Idaho, is seeking a Care Coordinator to manage a panel of members with chronic and complex health care needs. You will support clinical and non-clinical activities in a person-centered care coordination approach, collaborating with the care team to promote appropriate utilization and improved outcomes.

This remote-friendly role requires multitasking, attention to detail, and strong organizational skills.

Qualifications

• Idaho license required in one of: LCSW, LMSW, LSW, LCPC, LPC, LMFT, LAMFT, or LPN/LVN.
• 2+ years healthcare experience; Medicaid/Medicare or SDoH exposure preferred.
• Proficient in MS Office (Word, Excel, Outlook).
• Valid driver's license and ability to travel up to 50% within territory.
• Designated quiet home workspace with PHI security; reliable high-speed internet.

Responsibilities

• Serve as the primary care manager for dual-eligible members.
• Conduct comprehensive needs/wellness assessments and develop person-centered care plans.
• Collaborate with care team, providers, and resources to implement plans and remove barriers.
• Assist with obtaining HCBS supports and services.
• Coordinate care transitions following ER visits or facility stays.
• Educate and coach members for self-management and lifestyle changes.
• Advocate for member needs within the care team.
• Address nonclinical questions and connect to appropriate Health Plan resources.

Education

LCSW / LMSW / LSW / LCPC / LPC / LMFT / LAMFT license

LPN / LVN license

2-year degree or higher with 2+ years healthcare experience
