Responsibilities

• Coordinate intake and care plans to promote effective utilization.
• Support care team tasks and triage.
• Identify reasons for admission and refer to appropriate services.
• Coordinate services with providers within benefit plan.
• Support development and implementation of care plans.
• Promote internal/external communication to enhance medical management.
• Perform non-medical research for open cases.
• Provide support to team members.
