Formación

• 2-4 years of experience in healthcare field or medical setting.
• Experience in call center or customer service environment.
• Ability to work across multiple systems and tools efficiently.

Responsabilidades

• Coordinate care management activities and support care team intake.
• Screen and triage Care Team tasks and assist in care plan implementation.
• Identify appropriate referrals and coordinate required services.
• Support documentation and communication with providers and internal teams.
