Job summary

Humana is seeking a Care Coach to evaluate member needs and guide them toward resources that support optimal wellness. This remote role involves conducting outreach, assessing health needs, and coordinating services with a multidisciplinary team to improve outcomes.

Training for 4 weeks 8:30 AM to 5:00 PM EST, then hours are 10:00 AM–6:30 PM EST, Monday–Friday. Strong communication, EMR proficiency, and experience in direct adult care are required.

Qualifications

• Associate degree in any field.
• At least 2 years of experience in direct adult patient care.
• Experience interacting with members of the healthcare team.
• Strong computer proficiency and ability to learn EMR systems.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct telephonic outreach to members with chronic conditions.
• Assess member needs and develop individualized care plans.
• Coordinate services and connect to clinical and community resources.
• Identify and resolve barriers to care.
• Monitor progress and adjust care strategies.
• Maintain adherence to compliance in a regulated program.
• Navigate multiple computer systems in a fast-paced environment.
