Qualifications

• Bachelor's Degree with at least 2 years of relevant experience (Health Services, Social Work, Psychology).
• RN license in Florida with at least 2 years of relevant experience.
• LPN license in Florida with at least 4 years of relevant experience.
• Case Worker with a minimum of 6 years of Health Services/Case Management Experience.
• AHCA Fingerprint required.
• Valid state driver’s license and personal vehicle liability insurance.

Responsibilities

• Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long Term Care Facilities – 75-90% local travel.
• Assess and evaluate member's needs to establish a member-specific care plan.
• Ensure members receive services in the least restrictive setting to achieve well-being.
• Plan and implement interventions to meet those needs.
• Coordinate services and monitor the case management plan against member goals.
• Guide members/families toward resources for their care.
