Job summary

Humana Inc. is seeking a Care Coach to assess members’ needs and guide them toward appropriate health resources, with a focus on chronic conditions management. You will build individualized care plans, coordinate services, and monitor progress in a remote, structured environment.

The role requires strong communication, teamwork, and computer skills to navigate EMR systems and multiple platforms. Training is provided, and travel to Humana offices is occasional.

Qualifications

• Associate degree required.
• Minimum 2 years of direct adult patient care experience.
• Experience interacting with members of the healthcare team.
• Strong computer proficiency and ability to learn EMR systems.

Responsibilities

• Conduct telephonic outreach to members with chronic conditions.
• Assess member needs and develop individualized care plans.
• Coordinate services with clinical and community resources.
• Identify and resolve barriers to care.
• Monitor progress and adjust care strategies.
• Maintain adherence to compliance in a regulated program.
