Qualifications

• Associates degree in any field required.
• Minimum 2 years of direct adult patient care experience.
• Experience interacting with healthcare team.
• Strong computer proficiency, EMR systems familiarity.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct telephonic outreach to members with chronic conditions and barriers.
• Develop and manage individualized care plans through data review and conversations.
• Coordinate services and connect members to clinical, community and multidisciplinary resources.
• Identify and resolve barriers to care and member independence.
• Monitor progress and adjust care strategies.
• Ensure strict adherence to compliance in a regulated program.
• Learn and navigate multiple computer systems while engaging members.
