Humana is seeking a Care Coach 1 to evaluate member needs and coordinate care to support wellness and access to services. You will guide members and families toward resources, monitor progress, and help overcome barriers while adhering to privacy and regulatory standards.

The role requires bilingual ability (English plus another language), healthcare experience, and strong communication skills. Remote-friendly with home office privacy and occasional travel for training.

Qualifications

• Bachelor's degree in health and human services or LPN with IL license to practice.
• 2+ years of healthcare experience.
• Proficient with MS Word and Excel.
• Excellent verbal and written communication and interpersonal skills.
• Ability to use electronic information applications and software.
• Passion for improving consumer experiences.
• Must have a dedicated home office space with privacy.
