Humana is seeking a Care Coach to evaluate members’ needs and guide them toward resources for optimal wellness in a fully remote role. You will perform telephonic outreach, assess needs, and develop individualized care plans while coordinating with clinical and community resources to support positive health outcomes.

The position requires an Associate degree and at least 2 years of direct adult patient care, strong communication skills, and computer proficiency.

Competenze

• Associate degree in any course.
• Minimum 2 years of direct adult patient care experience.
• Experience interacting with members of the healthcare team.
• Strong computer proficiency and ability to learn electronic medical records.

Mansioni

• Conduct telephonic outreach to members with chronic conditions and barriers.
• Assess member needs and develop individualized care plans.
• Coordinate services and connect members to clinical and community resources.
• Identify and resolve barriers to care and independence.
